Maryland Hospitals for a Healthy Environment (MD H2E) invites you to participate in a survey developed in partnership with the Johns Hopkins Center for a Livable Future’s Maryland Food System Map Project. Together, we are examining the food system in Maryland to better understand how food moves from farm to hospital.
Your responses are important and appreciated. They will help us learn about purchasing practices of local and sustainable food, distribution within the healthcare sector, and allow us to celebrate and recognize your successes.
PRIVACY STATEMENT
We will maintain your utmost privacy with the information you provide in this survey. A summary of the information collected in this survey will be aggregated and used to report local food purchasing in healthcare. We will not report proprietary information on individual hospitals or those taking the survey. Any reports or maps created as a result of this survey will be shared with you and your organization first to ensure that your organization’s privacy is respected.

Your participation is voluntary and you may choose to skip any questions you do not feel comfortable answering. Our survey will take approximately 25-30 minutes. Please remember once you start the survey, you may come back to finish it later, BUT only by using the same computer. Consider printing this survey ahead of time by downloading the attached pdf – this will allow you time to gather your responses before entering them into the survey. Alternatively, you can complete the survey by hand and email it to the Map Project’s data specialist, Caitlin Fisher at cfishe29@jhu.edu.
